First Citizens Bank Exceptional Customer Experience Award Nomination Form
At First Citizens Bank, we value relationships. For more than a century, our customers have always come first. We’ve established the First Citizens Bank Exceptional Customer Experience Award to recognize those businesses that share our belief that delivering a great customer experience is paramount to a company’s success.
This award is significant in that it not only highlights the efforts of a small to medium-sized business that provides great customer service, but it also honors a company that has gone above and beyond in establishing an exceptional customer experience at every point of contact, throughout the organization.
CRITERIA |

Businesses must demonstrate innovative customer-focused ideas and strategies that go beyond their customers’ expectations and provide an overall exceptional customer experience. |

Companies in operation for at least three years with gross annual revenues not exceeding $10 million in 2008. |
| • |
Privately or publicly held corporation, proprietorship or partnership (not a subsidiary or division). |
| • |
Headquartered in Wake, Orange, Durham, or Chatham counties. |
| • |
Member of the Greater Raleigh Chamber of Commerce (financial services institutions, brokerage firms, and insurance agencies are not eligible). |

CEO/President actively participates in the daily operations of the company. |
JUDGING |

The Greater Raleigh Chamber of Commerce will select a panel of community and business leaders who will determine the award winner. The Award will be presented at the annual Greater Raleigh Chamber of Commerce Pinnacle Business Awards Celebration dinner, May 4, 2010. |
